About the Housing and Neighbourhoods Team

Our Housing and Neighbourhoods team deliver programs that Encircle implements on behalf of the Queensland Government, working in close cooperation with a range of other service providers, peer and community networks, and local governments.

In addition to managing three Neighbourhood Centres at Lawnton, Kallangur and Redcliffe, Encircle Housing and Homelessness activities have four main focus areas: Homelessness Support, Early Intervention and Prevention, Care Coordination and Service Navigation.

About the Homeless Outreach Access Program (HOAP)

The program covers Moreton Bay, this position is required to be based across the three Neighbourhood Centres, in addition to other locations.

To be successful in this role you will be able to utilise a strengths-based and recovery-focused approach, to provide outreach access to people experiencing homelessness in Moreton Bay. Support individuals or family households to build capacity to be independent, self-reliant, and connected to appropriate social and community supports with a focus on addressing housing needs.

In collaboration with the wider Housing & Neighbourhoods Team, meet all internal and external reporting requirements.

About this Role 

As a Senior Case Manager the role will contribute and lead the delivery of operational objectives for the HOAP team, utilising sound knowledge and skills gained through qualifications and experience.

Reporting to the Homelessness Programs Coordinator, the role will be responsible for supporting and guiding HOAP staff in the provision of assertive outreach case management support.

Provide leadership, support, supervision and advice to program staff and provide information to the Homelessness Programs Coordinator to make informed decisions in relation to complex client case management and service delivery issues.

Contribute to meeting HOAP internal and external reporting requirements including the collection and analysis of confidential data.

Key Selection Criteria

  1. Demonstrated ability to carry out needs identification, risk assessment/ management, referrals and advocacy.
  2. Demonstrated experience in team leadership, supporting and supervising program staff, problem solving, and driving change processes.
  3. Proven experience in delivering assertive outreach case management support for clients with high/multiple needs.
  4. Well-developed organisational skills that include the management of client records, database management, collection of statistical data and report writing.
  5. High degree of professionalism and well-developed interpersonal skills with a capacity to liaise effectively with a wide range of stakeholders including clients, service providers, volunteers, and staff.
  6. A high level of practice reflection and self-awareness.
  7. Demonstrated understanding of contemporary challenges, needs, issues and sensibilities of all people, including Aboriginal and Torres Strait Islander and those from culturally and linguistically diverse (CALD), LGBTIQA+ backgrounds and other vulnerable population groups and the application of Social Justice principles.

Required Qualifications and Experience

  • Must have a Social Work, Psychology or Human Service Degree combined with >1 year demonstrated experience in a Senior Practitioner role within a tenancy sustainment and/or homelessness context (preferred), OR
  • Diploma level Human Service qualification combined with extensive demonstrated experience (> 3 years).
  • Demonstrated experience in a leading/supervising staff, ideally within a housing and homelessness context.
  • Demonstrated experience in engaging with vulnerable population groups.
  • Experience in contributing to program budgeting and reporting.
